ANGEL TAG/TOY DRIVE

WHAT:          INTERVIEWS FOR THE ANNUAL SALVATION ARMY TOY DRIVE WILL BE HELD AT MARICOPA COMMUNITY CHURCH, 44977 W. HATHAWAY  
 
WHEN:  INTERVIEWS WILL BE HELD ON THE FOLLOWING DATES AND TIMES.
  • WEDNESDAY, OCTOBER 11TH FROM 6:00 pm to 8:00 pm
  • WEDNESDAY, OCTOBER 18TH FROM 6:00 pm TO 8:00 pm
  • SATURDAY, OCTOBER 21ST FROM 2:00 pm TO 6:00 pm
  • WEDNESDAY, OCTOBER 25TH FROM 6:00 pm to 8:00 pm
  • SATURDAY, NOVEMBER 1ST FROM 6:00 pm TO 8:00 pm
  • WEDNESDAY, NOVEMBER 4th FROM 2:00 pm TO 6:00 pm
 
 
WHAT YOU NEED TO BRING TO YOUR INTERVIEW:
  1. Proof of identification for the parents and each child receiving a gift. (12 years and under only)
  2. Identification may include: Birth certificate, AHCCCS card or immunization records.
  3. Proof of your Maricopa address.
  4. Last two months of a company W-2 or two month of current payroll stubs or food stamp paperwork. No hand written payroll information will be accepted.
  5. You will be asked to fill out an application with your name and spouse (if applicable) and each child that is 12 or under. (ONLY CHILDREN 12 AND UNDER ARE ELIGIBLE)
  6. Please be aware that you will need to list the child’s size of pants, shirts and shoes.
  7. Please do not bring children to the application process.

 
(Spanish speaking interviewer will be available)
 
Clearing House Statement
The Salvation Army will accept applications for individuals who are not participating in other assistance programs.  Please understand all information on the application is shared with other agencies in Pinal and Maricopa Counties.  If you apply with another agency, we will withdraw you from our program.
